These referrals must be based on medical conditions or symptoms that could affect safe driving and not the age of the driver. The report must be made in writing, giving the full name, date of birth, address, and a full description of the alleged physical or mental disability that could affect the driving ability of any person over 15 years of age. If information is missing or not included it will invalidate the form. The Medical Referral Form (72190 English / 72190 Spanish) should be used to report these concerns. Law enforcement officers can also refer a driver for reevaluation when completing an accident report. Reports will only be used to determine the qualifications of the individual to operate a motor vehicle on Florida’s highways. Second crash in a day? She may not have been impaired by DUI standards, but certainly would meet the requirement of having her license taken for the purpose of re-evaluation and examination, right? FHP could and should use this tool in the box to help make our roads safer, but you never read or hear about it.
